Home Hospital Services
The way Royal Perth Hospital (RPH) deliver home care to our patients has changed. The Hospital in the Home (HITH) service has transitioned to the Silver Chain’s Hospital at the Home (HATH) service.
Silver Chain is a not-for-profit community health and care organisation that supports people to live independently by offering a range of services to assist people in their homes. These services include specialist nursing, palliative care, home care and support services, hospital in the home and allied health services.
Short term acute services
Silver Chain provides short term acute home hospital services to patients across the Perth metropolitan area for a wide range of medical conditions.
Home hospital supports public hospitals by treating patients safely in the community, rather than in hospital. Patients are either seen in their own home or in a Silver Chain clinic. Treatment types include:
- intravenous antibiotics and re-hydration therapy via PICCs and infusor ports
- all interventions for patients in the immediate post discharge period
- complex wound care.
Home Antibiotic Service (HABS)
The new Home Antibiotic Service (HABS) at RPH is supported Monday to Friday by a clinic which is manned by a Silver Chain nurse. This clinic facilitates early discharge into the community.
Post-acute services
Silver Chain provides post-acute services such as wound care, continence management and stoma care. We also provide an industry leading palliative care service.
Home support
A range of aged care services are available through Silver Chain. Some patients may need to be referred to the Regional Assessment Service for an assessment in the first instance before they can start receiving aged care services through Silver Chain.
